Athletic Paperwork

If you wish to be a part of Ida S. Baker High School Athletics, you must fulfill the following obligations:
  1. Athletic paperwork must be done at Athletic Clearance.  
  2. Paperwork must be completed in full and signed by a parent/guardian in the presence of a notary.  There is a notary available at IBHS for your convenience from 7:00am-2:30pm during the regular school year.  There is no charge for this *service.  
  3. In order to abide by the Florida High School Athletic Association (FHSAA) which regulates interscholastic sports in Florida, every student must turn in a birth certificate before competing in interscholastic sports for the first time at IBHS.
  4. Every athlete must have accident insurance.  If you do not have insurance or would like additional coverage, please visit School Insurance of Florida.

The athletic office is here to assist you in becoming a Bulldog athlete; however, we cannot do your work. Make sure that all of your paperwork is complete and properly filled out. If it is not, you will not be cleared to play!! Athletes are not permitted to participate, even practice or condition (including summer workouts), unless all paperwork (including physical exam) is turned in. 

*It is illegal for a Florida notary to notarize a paper that is not completely filled out.
